Families must provide a description of the current household composition, such as whether the family lives independently, or with a relative(s) or others, is experiencing homelessness, or if the child(ren) living with them is a foster child(ren).

 

The relationship of any adult in the assistance group to the child(ren) in the assistance group must be verified by the child's birth records or other documentation necessary to verify the relationship of the child(ren) to the adult(s).

 

For a legal guardian relationship, require the court documentation *indicating the relationship of the adult to the child as that of a legal guardian.

 

For a caretaker relative relationship, require one or more of the following documentation:

a court order,

power of attorney; or

statement from the child's parent indicating that the individual is to provide care and supervision of the child(ren) on his/her behalf.

 

Verification of full time enrollment in high school or its equivalent must be provided for each child in the assistance group who is 18 or 19 years of age. Acceptable verification may include a signed and dated statement or other documentation of current enrollment from the school.
