943.01 Linking an Eligible Child to an Enrolled Child Care Provider (FAM)

The parent and enrolled child care provider must complete and submit a  Form 1863, Child Care Provider Verification within 10 calendar days of application. The FSS has 30 calendar days to determine eligibility and to link the child to the child care provider. If a link is not established to a DHHS enrolled child care provider within 30 calendar days of the date of application, child care will be denied, unless good cause is determined. Refer to Section 903 NH CHILD CARE SCHOLARSHIP APPLICATION PROCESSING TIME FRAME.

 

For expedited child care, a parent must identify a child care provider that is currently enrolled with the NH Child Care Scholarship program. Allow the child care provider to be linked at application without a Form 1863 if a parent is using a currently DHHS enrolled  child care provider when the FSS makes collateral contact with the provider to verify the correct Provider Resource Identification Number and that the child is scheduled to attend. A completed Form 1863 must be returned within 30 days.

 

For an open child care case, the parent must report a change in provider within 10 calendar days by submitting a new Form 1863. If after a change in provider, Form 1863 is received more than 10 days after the Child Care Link Date listed on Form 1863, establish the link between provider and child as the date Form 1863 was received by DHHS.

 

When establishing a new child care provider link, ensure the following on  Form 1863:

The form is completed with all pertinent information including the signatures of both parent and provider and dates; and

The parent and child care provider address is not the same, since the parent and child care provider cannot live in the same household and receive NH Child Care Scholarship.

 

When entering the child care provider link begin date at initial application, use the "Child Care Link Date " from  Form 1863 or the child care application date, whichever is most recent.

 

 Form 1863 should not be accepted more than 30 calendar days prior to the child's start date with the provider. Child care provider links cannot be established in New HEIGHTS with a future date of service.

 

Child care provider links may be end-dated when the worker is notified by the parent or child care provider that the child is no longer attending the program or when a new  Form 1863 is received. The child care provider end-date of the link is the last date that the child attended that child care program.

 

A new Form 1863 is not required at the redetermination if there is no change in the child care provider information.

 

If a redetermination is completed within 30 calendar days following the certification period, a new  Form 1863 is only required when there is a change in the child care provider information.

 

Each child may only be linked to one DHHS enrolled child care provider at a time. However, each child in the family may be linked to a different child care provider. If the family chooses to use more than one provider per child, the family shall be responsible for the full cost of care for the provider who is not linked to the child.
