901.01 Application Process for NH Child Care Scholarship, SR 17-12 Dated 07/17 (FAM)

he application process for Child Care Scholarship requires the individual to comply with all of the following:

• complete and submit a signed official application;

• participate in a personal eligibility interview;

• provide all required verification of eligibility criteria within timeframes; and

• indicate any circumstances that might change within the certification period which could affect the individual’s eligibility, such as changes in income, living arrangements, expenses, or child care provider.

Individuals can submit an application via NH EASY, fax, e-mail, mail, in person at the District Office, or over the telephone.

Personal interviews can be conducted in person or over the telephone.

Verifications can be mailed to the Central Scanning Unit, or submitted to the District Office in person, by fax, e-mail or mail.
