911 REDETERMINATION OF NH CHILD CARE SCHOLARSHIP SR 12-22, 04/12 (FAM-A)

NH Child Care Scholarship is determined for a specific period of time called the eligibility period. At the end of the eligibility period, NH Child Care Scholarship automatically terminates if the individual fails to complete a timely redetermination.

To complete a timely redetermination and continue NH Child Care Scholarship beyond the current eligibility period, it is the individual’s responsibility to comply with all of the following:

• complete and file an application at the District Office by the review date OR complete a review form and return it by the due date OR complete an online Redetermination Application via NH EASY by the due date;

• provide all required verification by the due date; and

• participate in a face-to-face eligibility interview, if required, by the review date.

Exception: Cases receiving another program of assistance in addition to child care assistance may be eligible to participate in the online or mail-in redetermination process, with no face-to-face interview required (see PART 161, REDETERMINATION/RECERTIFICATION, and Section 161.01, Timely Notice).
