711.01 Excessive EBT Replacement Card Notification (FSM)

All requests for replacement Electronic Benefits Transfer (EBT) cards are monitored. Upon the SNAP household’s fourth request for an EBT replacement card within a 12-month period, the EBT Administrator or designee:

·          Notifies the SNAP household that the household’s EBT account is being monitored for potential suspicious activity. Enclosed with this notification is:

- EBT Pamphlet, How To Use Your NH EBT Card, in case the reason for the excessive requests for EBT cards is due to the household’s confusion about how to use their EBT card; and

- BFA Form 77U, Community Resources, in case the reason for the excessive requests for EBT cards is due to the EBT cardholder’s need for additional services, such as mental health, substance abuse, or shelter services, and this need is impacting the household’s ability to retain their EBT card;

·          Monitors the recipient’s EBT account for potential trafficking or other suspicious activity; and

·          Refers the case to the Special Investigations Unit (SIU) if trafficking of SNAP benefits or other fraudulent activity is suspected.

·          The notification to the SNAP household:

·          Includes the number of cards requested and over what period of time;

·          Explains that the SNAP household’s EBT card account is being monitored for potential trafficking activity;

·          Includes information that explains a misuse or fraudulent use of benefits, federal and state penalties, and the possibility of referral to the SIU for suspicious activity; and

·          Provides contact information for the DHHS Customer Service Center, should the household have questions or concerns regarding the notification.
