Concord, NH – Commissioner Jeffrey A. Meyers of the NH Department of Health and Human Services (DHHS) has announced the team that will undertake a Sentinel Review of the discharge of a patient at New Hampshire Hospital and subsequent suicide on July 27, 2016.

The Sentinel Review team is comprised of:

  • Hon. James Duggan, retired NH Supreme Court Justice;
  • Kevin O’Brien, former Assistant Commissioner, NH Department of Safety, and Major, NH State Police;
  • Susan Wehry, MD, a psychiatrist in Vermont who formerly served as Commissioner of the Vermont Department of Disabilities, Aging and Independent Living;
  • Hon. James J. Leary, NH Circuit Court Justice, who organized and presides over the State’s first mental health docket; and
  • Jill A. Desrochers, Assistant Attorney General, NH Attorney General’s Office.

The group will conduct its review and report its finding to Commissioner Meyers. The review is conducted under the confidentiality provisions of RSA 126-A:4 and the Department’s Sentinel Review Policy. The Department plans to issue a report on the group’s recommendations when the review is completed.
