Open Enrollment for the Medicaid Care Management Program and for the NH Health Protection Premium Assistance Program

Once a year you or someone in your household can change your Medicaid Health Plan.

If you are enrolled with a managed care plan through the Medicaid Care Management Program, you have two choices in 2018:

  • NH Healthy Families
  • Well Sense Health Plan

If you like your managed care plan, you do not need to do anything - your coverage will continue. If you want to change from one of these managed care plans to the other, please go to NH Easy or call the Medicaid Service Center (NH Only) at 1-888-901-4999 (TDD Access: 1-800-735-2964), Monday through Friday, 8:00 to 4:00 p.m. ET no later than December 29, 2017 at 4:00 pm. Your new plan coverage will start on January 1, 2018.

If you are enrolled with a Qualified Health Plan (QHPs) participating in the NH Premium Assistance Program, please be advised that starting on January 1, 2018 the Plans available to you are:

  • Ambetter from NH Healthy Families - Ambetter Balanced Care 8: This is a statewide network.
  • Anthem: Anthem Silver Pathway X Enhanced HMO: This is a statewide network.
  • Harvard Pilgrim Health Care (HPHC) – ElevateHealth: This is not a statewide network. Not available in Belknap or Carroll Counties.

If you are enrolled with one of the plans listed above and you like your plan, you do not need to do anything - your coverage will continue. If you want to change from one of these Health Plans to the other, please go to NH Easy or call the Medicaid Service Center at (NH Only) 1-888-901-4999 (TDD Access: 1-800-735-2964), Monday through Friday, 8:00 to 4:00 p.m. ET no later than December 15, 2017 at 4:00 p.m. Your new plan coverage will start on January 1, 2018.

If you or someone in your household is currently enrolled with Harvard Pilgrim Health Care (HPHC) - New Hampshire Network Silver Premium Assistance Plan, you will have received a letter from HPHC letting you know that the New Hampshire Network Silver Premium Assistance Plan will no longer be available to you after December 31, 2017.

This does not mean you are losing your health care coverage under the NH Premium Assistance Program. This does mean that you need to pick one of the 2018 Qualified Health Plans listed above no later than December 15, 2017 at 4:00 p.m. If you do not pick a new plan, one will be assigned to you. Your coverage with the new plan will start on January 1, 2018.

If you or someone in your household is currently enrolled with Minuteman – MyDoc HMO Silver Assistance Plan, you will have received a letter from Minuteman letting you know that the MyDoc HMO Silver Assistance Plan will no longer be available to you after December 31, 2017.

This does not mean you are losing your health care coverage under the NH Premium Assistance Program. This does mean that you need to pick one of the 2018 Qualified Health Plans listed above no later than December 15, 2017 at 4:00 p.m. If you do not pick a new plan, one will be assigned to you. Your coverage with the new plan will start on January 1, 2018.

To select a new Qualified Health Plan go to NH Easy, or call the Medicaid Service Center at (NH Only) 1-888-901-4999 (TDD Access: 1-800-735-2964), Monday through Friday, 8:00 to 4:00 p.m. ET.
