The New Hampshire Department of Health and Human Services has issued a plan for new models of delivery and payment for nursing facility and Choices for Independence (CFI) waiver home and community-based long term care services. The Implementation Plan for Medicaid Care Management - Nursing Facility/Choices for Independence will offer a choice between managed care and locally based and operated PACE Centers.

March 6, 2018 Notice

Notice is hereby given that the New Hampshire Department of Health and Human Services is seeking public input on the Implementation Plan for Medicaid Care Management - Nursing Facility/Choices for Independence Services in adherence to requirements per Senate Bill 553, enacted in June 2016, as established in Revised Statutes Annotated (RSA) 126-A:5, XIX, which required the Department to engage stakeholders to develop an implementation plan for Phase 2 of Medicaid managed care specific to nursing facility and home care long term care services and supports.

In order to re-balance long term services and supports away from unnecessary institutionalization in nursing home facilities and to improve care coordination and quality, and to implement a payment system that emphasizes value and not volume, the Department is proposing a hybrid model for the delivery of NF and CFI services in New Hampshire, these long term care services in New Hampshire: a managed care model that will be delivered through health plans contracted by the State and locally established programs for all-inclusive care for the elderly, or PACE Centers.

Public Hearing Postponed to March 27, 2018, 6:00 – 8:00 PM
The Department of Health and Human Services will hold a public hearing on the on the Implementation Plan for Medicaid Care Management - Nursing Facility/Choices for Independence on Tuesday, March 27, from 6:00 – 8:00 PM at the NH Department of Health and Human Services, Auditorium, 29 Hazen Drive, Concord, NH.
