How do I get reimbursed for mileage to NH Medicaid Services?
Enroll in the Friends and Family Mileage Reimbursement Program: You will be paid for your mileage from your home to your Medicaid-covered healthcare service and back to your home. If you are unable to drive yourself, friends and family can enroll in the program to be paid for mileage to take you to healthcare services. You must call CTS in advance of the trip if you want to be reimbursed for mileage.
If you are enrolled in the Friends and Family Mileage Reimbursement Program, you need to complete a Medicaid Transportation Reimbursement Form for each approved trip.

Print the Medicaid Transportation Reimbursement Form, fill in the top part of the form, and bring it to your medical appointment, because the provider has to complete and sign the bottom part of the form. You then need to mail, scan and email, or fax the completed form to CTS as specified on the form.
How do I request a ride to NH Medicaid Services?
Medicaid recipients who do not have a vehicle or a friend/family member who can drive them can request a ride by calling CTS at toll free number: 844-259-4780. CTS will arrange the ride using public transportation, a transportation service, a wheelchair van or non-emergency ambulance service if medically necessary. Please call at least 48 hours in advance to request a ride.
Please have your Medicaid ID number, which is on your blue Medicaid card, and appointment information available when you call CTS.
