Philbrook Adult Transition Housing (PATH)

PATH logoPhilbrook Adult Transitional Housing (PATH) is a 16-bed voluntary housing unit that provides services to individuals who have been released from inpatient psychiatric care and can live independently, but need assistance to obtain permanent housing.

PATH opened on September 14, 2020, and in the first year served 58 clients, 81% of whom were on conditional discharges from New Hampshire Hospital. Average length of stay for those discharged from PATH varies between 60-80 days.

Please feel free to reach out to PATH if you have any questions about the program or admission criteria.

"I wanted to take the time to write to you in support of the PATH program. There really aren't words to explain how very difficult and frustrating the past 2 years have been for my family as my son's illness began to present itself. I have been told he was a drug addict or just plain selfish and lazy. Told at the hospital he wasn't sick enough for inpatient and sent to jail instead. He sat in jail for months on end with no treatment. I could go on but you have probably heard these stories before.

My main point in writing to you today is to express my extreme gratitude for the staff at NHH and now for the assistance of PATH case managers. I can tell they are both very dedicated to their work and they make things so easy for a very tired and heartsick mom. I am sure you get many negative emails so here is one to simply say, I think this program is fantastic and I thank God every day that we were fortunate enough to find it.

I hope that you have a great day, please also give my regards to all of the staff at PATH." ~ Family member

"I want to begin by thanking you and yours for all the help I have received over the course of the last few months! I will try and address the joys that everyone has brought me.

  • My counselor has been kind and positive since I first arrived at PATH.
  • My case manager has been a godsend.
  • The nursing staff has been helpful and informative.
  • Your kitchen staff is exceptional. I am a vegetarian and the kitchen staff has been accommodating and patient.
  • Mental Health Workers always saw to it that I made it to appointments and to the stores when I needed it. They are unsung heroes.
  • May God bless and prosper you in your future endeavors." ~ Former PATH client

“Thank you so much for all your help. I truly appreciate all the hard work you did to not only get me in the Bridge program, but into a two bedroom apartment. Please tell everyone I miss them!” ~ Former PATH client
