File a Grievance with the Bureau of Adult & Aging Services
How Choice for Independence (CFI) participants can report grievances to the Bureau of Adult & Aging Services (BAAS)

“Grievance” means an expression of dissatisfaction or complaint on behalf of a participant related to the department’s administration of CFI or a provider’s performance of CFI services regardless of whether remedial action is requested.
Who Should Report a Grievance
Participants have the right to report grievances. Grievances can be reported by any individual including but not limited to:
- The participant
- Participant’s guardian
- Participant's family member
- Employee
- Contractor
- Consultant or volunteer for the Department of Health and Human Services (DHHS)
- Choices For Independence (CFI) provider.
To Report a Grievance
Grievances can be submitted by using BAAS Form 3123 or BAAS 3123: Informe de quejas, by writing a statement, or by verbal submission, such as a phone call. Grievances can be reported and submitted to BAAS in the following ways:
- Email BAAS Form 3123 or BAAS 3123: Informe de quejas, a written statement, or provide a statement in the body of the email, to BAASQualityManagement@dhhs.nh.gov;
- Mail the grievance through the USPS using BAAS Form 3123 or BAAS 3123: Informe de quejas or provide a written statement to:

- Verbalize the grievance by telephone to 603-271-2240; or
- Fax the grievance using BAAS Form 3123 or BAAS 3123: Informe de quejas or a written statement to 603-271-4643, Attn: Quality Coordinator.

When you report a grievance, you will be asked for some information. You may not have all the answers. Just tell us what you know regarding the grievance.

Complaint Resolution
The BAAS Quality Coordinator may be able to reach resolution of a grievance with, or without an investigation.
The Quality Coordinator will present BAAS management with findings of an investigation, recommended resolution and suggested changes, if applicable.
Confidentiality
The grievance record shall be confidential and shall not be disclosed without consent of the individual or group of individuals on whose behalf the grievance is made, except as necessary to assist in the resolution of the grievance, or as required by law.
