Submit a Floor Plan Review
As part of the licensing process for applicants of newly constructed or remodeled food establishments, you must submit a Floor Plan Review Application to Food Protection.
Plans are reviewed to ensure that the establishment will have the necessary facilities and equipment required to be licensed.Before you start, please review the Food Establishment Floor Plan Submittal Tip Sheet.
To submit a Floor Plan Review Application online, you will need to register with Food Protection’s Online Portal. These detailed instructions will walk you through a step by step process to register online. Please include with the online application the completed Food Operation Questionnaire.
Alternatively, if you prefer, you may choose to submit a paper application form.
