Join the NH Care Connections Provider Network

This form allows interested people to request information about joining the NH Care Connections Network

The NH Department of Health and Human Services (DHHS) and the NH Department of Military Affairs and Veteran Services (DMAVS) are excited to partner with Unite Us and PointClickCare. The NH Care Connections initiative aims to connect communities across the State of New Hampshire for improved coordinated patient care and better health outcomes for the residents of New Hampshire.

The process of joining the NH Care Connections Network begins with contacting DHHS. The department’s dedicated NH Care Connections team will review your request and schedule some time to discuss it with you.
