Relocation of Portsmouth District Office #6 and New Name for the Office: Seacoast District Office

EFFECTIVE DATE:

SUMMARY

 

This SR releases the relocation and renaming of the Portsmouth Division of Family Assistance District Office #6, from the Portsmouth District Office 30 Maplewood Avenue, Portsmouth, NH, to the Seacoast District Office, 50 International Drive, Portsmouth, NH.

 

POLICY

 

Effective April 16, 2010, the new name and address for District Office #6 will be:

 

State of New Hampshire

Department of Health & Human Services

Seacoast District Office

50 International Drive

Portsmouth NH 03801-2862

 

Phone numbers for the Seacoast District Office, including fax lines, remain unchanged.

 

NEW HEIGHTS SYSTEMS PROCEDURES AND IMPLEMENTATION

 

Effective April 16, 2010, New HEIGHTS will begin using the new name and address on all correspondence (NODS, letters, etc.) generated for District Office #6.

 

IMPLEMENTATION

 

The Seacoast District Offices new address becomes effective April 16, 2010. Mail will be forwarded from the old address for at least a year, so the current Business Reply envelopes with the old address will continue to be used until the supply is exhausted as a cost-savings measure.

 

CLIENT NOTIFICATION

 

A one-time mailing to notify all open cases assigned to the Seacoast District office of the new address will occur the week of April 12, 2010. The letter is attached for reference. No other client notification is planned.

TRAINING

 

No training is planned or needed.

 

DISPOSITION

 

This SR may be destroyed or deleted after its contents have been noted and the SR has been posted to the On-line manual.

 

DISTRIBUTION

 

This SR will be distributed according to the electronic distribution list for Division of Family Assistance policy releases. This SR will be available for agency staff in the On-Line Manual Library, and for public access on the Internet at http://www.dhhs.nh.gov/DHHS/DFA/LIBRARY, effective May 3, 2010.
