SUMMARY

 

This SR releases revised Form 226, Initial Employment Appraisal, and Form 226(i), Instructions for Form 226.

 

POLICY

 

Form 226, Initial Employment Appraisal, is a two-part NCR form used at the district office during the eligibility interview for an assessment of the client’s skills, prior work experience, and employability. One copy is kept by the district office and one copy is sent to the NHEP Team. Federal law now requires an initial assessment of every TANF financial assistance applicant. Form 226 is an important part of this mandated assessment.

 

Form 226 must be completed:

 

 At the initial eligibility interview, for every TANF financial assistance applicant who is not obviously ineligible and who is a potential mandatory or voluntary NHEP participant (i.e., all included parent/caretaker relatives and children 16 or older who are not full-time students), and

 

 At the redetermination or other contact, for every TANF financial assistance recipient who:

 Volunteers for the NHEP program;

 Has a change in status from exempt to mandatory or voluntary;

 Enters a self-initiated education/training and did not complete the Form 226 previously.

Note: A new Form 226 is not necessary when there is a Form 226 less than 90 days old.

 

Form 226, Initial Employment Appraisal, was previously titled Initial JOBS Appraisal. This form has been revised and reformatted with input from the NHEP Team members. The revisions were intended to make it more user-friendly and focused, and to better reflect the "employment first" philosophy of the State’s welfare reform program.

 

IMPLEMENTATION

 

Initial supplies of revised Form 226 will be available on or about March 1. Begin using the new Form 226 as soon as they are available from Logistics. At that time, destroy all old copies of Form 226. A Certificate of Destruction is attached to be completed and returned to State Office.

This SR may be destroyed after its contents have been noted and its posting instructions carried out.

 

DISTRIBUTION

 

This SR only will be distributed according to the narrative distribution list for the Family Assistance and Forms Manuals. A copy of the Form and Instructions will be distributed to all holders of the Forms Manual.
