Health Professions Surveys
Pursuant to RSA 126-A:5, XVIII-a, the following New Hampshire-licensed health care professionals are required to complete the Health Professions Survey or Opt-Out Form as a license renewal requirement in the State of New Hampshire.

Providers that had a temporary license converted to an active status license on 4/3/2024 with license renewal due by 6/30/2024 are exempt from the survey requirement.
Open Provider Surveys
NH Alcohol and Drug Counselor Licensure Survey
The New Hampshire Alcohol and Drug Counselor Licensure Survey is now available to complete for licensees due to renew their license in 2024. Only NH-licensed LADCs/MLADCs with an active status license expiring June 30, 2024 will have access to the survey.
Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

NH APRN Licensure Survey
The New Hampshire APRN Licensure Survey is now available to complete for licensees due to renew their license between 7/1/23 and 6/30/24.
Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.
Licenses issued after the start of the respective survey cycle (7/1) are exempt from the survey requirement.

NH Dentist Licensure Survey
The New Hampshire Dentist Licensure Survey is now available to complete for licensees due to renew their license in 2024. Only NH-licensed dentists with an active status license expiring April 30, 2024 will have access to the survey.
Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

The New Hampshire Mental Health Practitioner Licensure Survey is now available to complete for licensees due to renew their license between 7/1/23 and 6/30/24.
Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

The New Hampshire Physician Assistant Licensure Survey is now available to complete for licensees due to renew their license in 2023. Only PAs due to renew their license by December 31, 2023 will have access to the survey.

The New Hampshire Psychologist Licensure Survey is now available to complete for licensees due to renew their license in 2024. Only psychologists due to renew their license by June 30, 2024 will have access to the survey.
Survey access is restricted to psychologists with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement. School psychologist licenses are exempt from the survey requirement.
