Open Provider Surveys

NH APRN Licensure Survey

The New Hampshire APRN Licensure Survey is now available to complete for licensees due to renew their license between 7/1/23 and 6/30/24. 

Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

APRN licenses issued after 7/1/23 are exempt from the survey requirement.

NH Dentist Licensure Survey

The New Hampshire Dentist Licensure Survey is now available to complete for licensees due to renew their license in 2024. Only NH-licensed dentists with an active status license expiring April 30, 2024 will have access to the survey.

Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

NH Mental Health Practitioner Licensure Survey

LICSWs, LCMHCs, LMFTs, LPPs

The New Hampshire Mental Health Practitioner Licensure Survey is now available to complete for licensees due to renew their license between 7/1/23 and 6/30/24. 

Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

NH Physician Assistant (PA) Licensure Survey

The New Hampshire Physician Assistant Licensure Survey is now available to complete for licensees due to renew their license in 2023. Only PAs due to renew their license by December 31, 2023 will have access to the survey.

Survey access is restricted to providers with a current, active status license. All other license statuses (temporary, inactive, etc.) are exempt from the survey requirement.

Closed Provider Surveys

• NH Alcohol and Drug Counselor (LADC/MLADC) Licensure Survey
  The survey will open May 1 and close on June 1, to correspond with license renewal under the Board of Licensing for Alcohol and Other Drug Use Professionals (330-C:22)

• NH Dental Hygienist Licensure Survey
  The survey will open March 1 and close on May 1, in odd-numbered years, to correspond with license renewal under the Board of Dental Examiners (317-A:13)

• NH Physician Licensure Survey
  The survey will open May 1 and close on June 30, to correspond with license renewal under the Board of Medicine (329:16-a)

• NH Psychologist Licensure Survey
  The survey will open May 1 and close June 30, to correspond with license renewal under the Board of Psychologists (329-B:12)
