This Request for Applications (RFA) is published to solicit applications from community-based organizations to support older adult and disability populations throughout New Hampshire by providing transportation services to and/or from an individual’s home to a specific destination, which may include, but is not limited to:
- Medical/Dental Appointments
- Shopping
- Socialization
- Community Dining/Congregate Meals
- Volunteer opportunities
