This Request for Applications (RFA) is published to solicit applications from licensed entities and organizations to provide In Home Care Services and Home Health Aide Services to support older, isolated and frail adults, age 60 and older, to live as independently as possible, safely, and with dignity, and to adults between the ages of 18 and 59 who have a chronic illness or disability.
