Division of Long Term Supports and Services (DLTSS)
This Request for Proposals (RFP) is published to solicit proposals for the provision of specialty services for children with medical complexity from birth to21 years of age. The selected Vendor will provide services to:
- Enhance comprehensive care for Children with Special Health Care Needs (CSHCN) whose access to appropriate care is complicated by disability, economic, cultural, linguistic, and/or other social/structural barriers;
- Increase the participation of families with CSHCN in planning and delivery of services in all programmatic areas, including the assessment of consumer satisfaction; and
- Enhance the ability of established organizations linked to communities to provide quality health care and family support to CSHCN.
