This Request for Proposals (RFP) is published to solicit proposals for Care Coordination services in order to assist Family Caregivers of individuals caring for individuals with Alzheimer’s disease and related disorders (ADRD), as defined in He-E 503, with Statutory Authority RSA 161-F:66, I, and are receiving care in order to remain in their homes as independently as possible in safety and with dignity.
