Division for Children, Youth and Families
The New Hampshire Department of Health and Human Services is seeking responses to this Request for Proposals (“RFP” or “Solicitation”) from qualified vendors to provide Community Navigator outreach services to families who do not rise to the level of being assessed by the Division for Children Youth and Families (DCYF), by educating professional reporters and families on available resources in their specific region and assisting families with navigating referrals to specific resource(s) in order to connect families with community supports and services.
