The New Hampshire Department of Health and Human Services is seeking responses to this Request for Proposals (“RFP” or “Solicitation”) from qualified Vendor(s) to provide home visiting services to pregnant individuals, and families with children up to age five (5), by utilizing the evidence based home visiting model from Healthy Families America (HFA) and Child Welfare Protocols.
