The New Hampshire Department of Health and Human Services, Division of Long Term Supports and Services (“Department”) is seeking responses to this Request for Proposals (solicitation) from qualified Vendors to facilitate the scheduling and completion of the Supports Intensity Scale-Adult® (SIS-A®) assessment for all individuals who are receiving, and who plan to receive services through the New Hampshire Developmental Disability 1915(c) waiver in accordance with He-M 500, part 503 and part 522.
