Immunization Guidance for Schools
New Hampshire Administrative Rule He-P 301.13-15 requires that all children enrolled in any school, pre-school, or child care have certain immunizations to protect them and those around them from vaccine preventable diseases. In addition, schools and child care providers must collect and review the immunization records of enrolled children and submit an annual immunization report to the DHHS (RSA 141-C:20-e).

COVID-19 has disrupted both in-person learning and routine well-child visits for many children over the last year. As a result, many children have fallen behind on receiving recommended vaccines. We all want our kids to stay in school safely and that means getting caught up on vaccines that were missed. Health care providers are prepared to safely give children the vaccines they need. Encourage parents/guardians to contact their doctor to be sure you and your family are up to date on all recommended vaccines.

NH Schools Guide to Immunizations
The “NH Schools Guide to Immunizations” has temporarily been removed from the DHHS website as it undergoes revisions. - Each year, polio vaccine requirements are grade progressive. See the school immunization requirements for details.

Annual School Immunization Report
Each year, every public and non-public school in New Hampshire is required to complete and submit the Annual School Immunization Report by November 15th.
The electronic school report link is emailed to schools in late summer.
