Provider and Area Agency Staff Survey - 3/12/2024
BDS values your input! Your engagement in our surveys contributes to the strengthening of the developmental services system. By sharing information and ideas, you support efforts to enhance provision of and access to quality services.
The survey comes in two versions: one tailored for employees of Area Agencies (including if you are a service coordinator or direct service provider at an area agency) and another designed for employees of service coordination and service provider agencies. Please follow the instructions below.
- Area Agency Staff Survey - Please complete this survey if you are an employee of an Area Agency (including service coordinators and direct service providers at an Area Agency). You should not complete the Service Coordination Entity and Service Provider Agency Staff Survey.
- Non-Area Agency Service Coordination and Service Provider Agency Staff Survey - Please complete this survey if you are an employee of a service provider or service coordination agency. You should not complete the Area Agency Staff Survey.
These surveys will close on 4/12/2024.
We'd also like to inform you that the Bi-Weekly Provider Readiness Meeting scheduled for March 26th from 11-12pm will be a listening session. Our objective is to seek your input and feedback as an employee of a service provider agency and use that feedback to collectively shape the future of the developmental services system.

Becoming a Medicaid-Enrolled Provider in New Hampshire
Under federal regulations from the Centers for Medicare and Medicaid Services (CMS), providers must have the option to bill for services directly through the State’s Medicaid Management Information System (MMIS) or use a third-party biller.
All providers providing services, including all Area Agencies, for individuals under the Developmental Disabilities (DD) Waiver, In-Home Supports (IHS) Waiver, and/or Acquired Brain Disorder (ABD) Waiver must be a Medicaid enrolled provider, whether they intend to bill directly or not.
- Visit NH’s MMIS Health Enterprise Portal to submit an application
- Entities interested in being a trading partner will also need to go through the enrollment process
